- STEP1
Call a contracted health screening institution for a reservation
You should make a reservation at a contracted health screening institution by phone in advance. When doing so, you should notify them of the following two points.
[1] You are an insured person of the Advantest Health Insurance Society or his/her dependent.
[2] Benefit One Inc. will serve as an agency for your reservation.Courses, fees, and other matters specified by contracted health screening institutions can be found on “Happylth Health Screening reservation website” located in Health Portal website.

Apply for a subsidy by providing your reservation details through “Happylth Health Screening reservation website” located in Health Portal website(application for subsidy)
After you have made a reservation, you should apply for a subsidy through “Happylth Health Screening reservation website” located in Health Portal website by providing your reservation details.

- STEP3
Receive a letter of acknowledgment of reservation, test kits, medical interview form, etc.
You will receive a notification of the definitive reservation from the health screening reservation reception center.
*For the online application, you will receive a “reservation completed” or “reservation application completed” email.
*For the phone-call application, a “letter of acknowledgment of reservation” will be sent to you by postal mail.Around one week prior to the day of examinations, test kits and other materials will be sent to your home from the health screening institution. You should be prepared for examinations as instructed by the health screening institution.
*Whether you will receive pre-supplied materials or not may vary depending on the health screening intuition. - STEP4
On the day of examinations
You should visit the health screening institution with which you made your reservation by bringing the following items.
- Health Insurance card
- Items to be submitted as requested by the health screening institution (a urine collection container and other test kits, medical interview form, etc.).
*Health screening tickets issued by the old system are no longer offered.
You will be required to pay your co-payment amount of the health screening fees at the reception desk of the health screening institution. - STEP5
Notification of health screening results
You will receive a notification of your health screening results from the examiner health screening institution in one or two months after you receive examinations. Your health screening results will be sent to the address you have registered with Happylth Health Screening reservation website, and you should make sure to notify the health screening reservation reception center if your registered address changes after your reservation becomes definitive.

- STEP1
Make a reservation with a non-contracted health screening institution
You should make a reservation at a non-contracted health screening institution in advance. The information about examination items which must be satisfied to be covered by subsidies can be found here.
- STEP2
Report your reservation details to the Health Insurance Society
After you have made a reservation, you should download a “Memo on Reservation for Complete Check-up, etc. (for Non-contracted Hospitals),” fill out the form with the necessary information, and submit it to the Health Insurance Society.
If you take examinations at a non-contracted health screening institution, you are not required to make an application through “Happylth Health Screening reservation website” located in Health Portal website.
- STEP3
On the day of examinations
You will be required to pay the full amount of the health screening fees at the reception desk of the health screening institution.
- STEP4
Application for payment of a subsidy (ex-post application)
Application for payment of a subsidy will become available after you take the examinations.
After receiving the result sheet issued by the health screening institution, please follow the application process shown below.You should download an "application for a subsidy of complete check-up, etc. (for non-contracted hospitals)" and "medical interview form for specified health diagnoses," fill out the form with the necessary information and place your seal on the form, and submit it together with attachments to the Health Insurance Society.

[3] Medical interview form for specified health diagnoses (this form is not required when you take a brain checkup or gynecological exams only)
*Please make sure to have your receipt issued in the name of the examinee as the addressee of the receipt.
After you have applied for a subsidy, you can check KOSMO Web (medical care cost information system) for the status of the determination of eligible subsidy recipient.
