If an insured person is unable to work to undergo treatment for a nonoccupational sickness or injury and is unable to earn salary or other income during this time, he or she will be paid “Injury and Sickness Allowance”.

Deadline:	As soon as possible
Applies to:	Insured persons taking time off from work due to sickness (those satisfying all four conditions below)
Submit to:	AT: SATO Labor and Social Security Attorney Office (Through e-pay Portal application, we will send the procedural documents to your home.) Other than AT: social insurance personnel of each company(Administration Department) * Submit the form to the Health Insurance Society through the employer
Notes:	You must satisfy all four of the following conditions to receive this allowance: You must be undergoing treatment for the sickness or injury You must be unable to work due to the treatment You must have missed work for at least three consecutive days You must be unable to earn your salary or other income
